HMS support people to take control of their life, we believe that everyone has the right to live independently and to be included in the community. We strive to promote, protect and fulfil the rights of people learning disabilities and/or mental health issues to live independently in the community and to secure their full inclusion and participation in all aspects of life. HMS ensure that individuals have the opportunity to choose where and with whom they live, and who provides their support. We support people to plan their lives and manage the support they require to achieve their dreams and aspirations. We work in partnership with professionals to support people to live, learn, work and achieve their life goals. We support people to become part of a community and maintain and build relationships, maintain their home, tenancy and all aspects of their daily lives. This includes their health and well-being, safety and security, political participation, leisure and hobbies, employment opportunities, accessible travel and access to goods, services and public spaces.
We offer personalised floating support to adults with learning disabilities, mental health needs or any other needs, to maintain or improve independence within their homes and to provide support to access the community to enable individuals to achieve their goals, dreams and aspirations. Our services focus on recovery, wellbeing, choice and greater independence. We enable people to make progress in these areas by recognising and building on their strengths, coping abilities, social networks and natural support systems.
